Rear extension and general refurb work - What order?

Im thinkin about a rear extension to the house which will be a kitchen with the existing kitchen being turned into an open plan dining area off the new kitchen. I also want some general refurbishment work such as new bathroom, skimming of one bedroom wall and new wardrobes fitted, the possiblity of removing the plaster from hall, stairs and landing and redoing, a new boiler and extension of the central heating for the extension, and new double glazing.
Its mostly cosmetic work really but wondering the best way to go about it all.
One builder for everything? Keep the extension seperate from the rest and have seperate contractors? Do extension first, then someone to fit kitchen?

Just looking at the best way to organise it and order of works.

An advice will be appreciated.
