Repair Kitchen Screed

I have to Replace some Kitchen floor Screed that was crumbling, the area is about 3x3 sq metres existing screed thickness is 40mm, i could do with some positve and helpfull advice on repairing this.The floor is block and beam with polystyrene insulation on top.
 
On top of the block and beam should be Dpm of 1200 to 1400 guage,Then should be insulation of minimum 80mm of Kingspan or similar, then a vapour barrier topped off with 75mm screed [ reinforced or not ]dependant upon foot traffic , not quite sure what your describing but it don't sound right.

Thanks for the reply,didnt see any dpm just block and beam polystyrene and screed,i can send you a pic if you are interested to see it. it looks a mess thoigh where i tried to piece back the styrene,also this is the upstairs of the house not ground floor all rooms are upstairs i think it is known as a flying freehold,ie my house is over my garage and my neighbours either side of me.
