Is it possible to replace a standard size terrocota air brick and sleeve with a double air brick/sleeve from the OUTSIDE of the building only? That is to say NOT lifting the floorboards and being able to work at it from "both ends" so to speak.

Yes ... if you make a big enough hole to be able to access the inner leaf .... but it will be so awkward if you are talking about trying to bend down and work so close to ground level
