Been filling holes then sanding but as in photo white from filler has come through first coat. Do I need to sand all filler back except stuff intended holes or lines?
The filler is sucking in the paint, that's why it shows through. You'd normally put a mist coat on new plaster, but in your case, it'll juts be putting on a few more coats there to even it up.

Shall I scrape out the flaky bits then put caulk in? Regarding the filler, is it best to water down paint so a primer then use zinza on top of that or use zinza straight onto the fresh filler?
If you scrape out any loose bits, then the caulk will grip better. When you get some new filler done, use the 50/50 emulsion and water to seal it first, before you paint the whole wall.
Sorry, but as I never use the Zinsser, so I can't help you on that last one.
