You need to connect the 240v side of the transformer in parallel to the connections you made for the switched live and neutral for the fan, but on the input side of the fan isolator. Then the 12v leads go to the lamp.

If that's too difficult, you can take the switched live and neutral straight from the fan terminals (or by using a junction box if necessary) to the transformer. This would mean that when you turn off the fan isolator for whatever reason, you will also lose the light.

Alternatively, run a new cable from another light in the room.
