Supporting cables with clips?

In Appendix 4 of the OSG on page 103 it tells you where to clip the cable for vertical and horizontal runs. It gives the value of the cable diameter in mm. The first one is not exceeding 9mm, do I just measure the width of the cable and use that figure?

Is this for if the cables are running free along a wall and accessible?

What if the cables are running between the ceiling/floorboards between floors. Do these have to be clipped or can they be left free running along the joists and resting on the ceiling below? If they do have to be clipped, how often do that have to be clipped? The same goes for cables running in a loft and if they have to be clipped?

Can cables also take the shortest route when they are between floors or in the loft, or do they have to conform to some sort of routes like they do when they are embedded in walls?
