Tap: any idea of the problem?

Morning!
Please see pic of tap. It's a lever tap - but it's just started running constantly. Moving the handle alters the flow but more in a random sense.
1) could it just be that it needs a new washer?
2) if it's the washer, what kind do I ask for at the plumbers merchant? (size / spec)

It looks like a 1/4 turn ceramic insert type of tap - its advisable to take the insert with you when you go searching as the new one must be exactly the same. The likes of B&Q etc have some types, but there are dozens available.

...thanks..it is a 1/4 turn. I needed o buy a part then fit it though - if possible...rather than turn off the water, take it apart, then go to the shop.
...but by the sound of it, this is not an option.
