Tiles in bathrooms again!

I have a bathroom floor covered in carpet, rock board underneath as per modern construction.
If I board 18mm ply over that instead of carpet its going to be very thick, without the floor tiles and adhesive being laid on top? The door would never open and would be a step into bathroom.
Whats the solution?
 
Nothing wrong with a small step up into a bathroom. Mine all have a step.

Get a nice bit of oak made up for the threshold then have the bottom of the door shaved to fit.
