I have changed this twice before and both times it’s always been a piece of cake and no issues. However now when I changed the broken flush handle it’s installed the new one it’s still very loose. A slight gap between the nut and the cistern and also the metal rod in between is really loose and it can move in and out. Any ideas on how to fix this ?

What about the one you removed?
Drill a 3mm hole near the sleeve, add plastic washer and brass split pin.
