Spend less on shareholders and more on preventative maintenance?

Send out crews to examine equipment more frequently?

That would obviously help, although one has to remember that, in the absence of attracted shareholders, there would probably be little or no preventative maintenance and quite possibly no service

Again, that would help. However, it only takes 'moments' for an incident such as we are discussing to kill, and they obviously cannot send out crews 'every moment' to every inch of their network - so, as I implied, I think there is always inevitably going to be considerable reliance on reports from the general public in relation to that sort of problem.

And it only takes moments to replace parts. More regular routine replacement of parts known to wear out should help to prevent such incidents happening in the first place.

It didn't.

18 months to the inquest, because presumably a prosecution can't be considered until the manner of death is official, and then 2 years until the court case.

You're getting the two incidents mixed up.

In the earlier incident they were aware.
