Water Ingress to Lintel

How do you know it is water ingress rather than condensatioin, which it looks like. Is the outside of the wall ok. Can you see any broken/missing roof slates/tiles above the area
If the outside is ok don't worry about it just fill and decorate and improve the heat and ventilation in the bathroom
