What is the cause of this issue?

Does anyone know what might be going on here. This is a small area of an otherwise good plaster wall. It’s been filled twice and twice it has come away, generally in cold weather.

The centre of it almost looks like the concrete block of the wall and it almost seems like there is something in that particular block that is causing this.

The whole thing is about 2-3” in diameter.

It is on an outside wall. There is nothing obvious on the outside and the remainder of the wall is all ok.

Any ideas what’s going on here and how best to deal with it so it doesn’t keep causing a problem.

Could if been a lump of dry sand on the block when skimmed over.
Never really worked out why.
I've seen a few times and just filled in after vacuum blocks clean of dust
