Where does the DPM go?

Hi I’ve got a single storey extension that now has a roof and windows. I am needing to lay a DPM on the concrete slab ready for 100mm insulation and then screed that will tie in with the lounge that has been knocked in to.

My question is... how do I join the new DPM with the existing house? Hopefully the pictures will explain this much better than I can. Thanks!
