i would have thought the answer to both posts 1 and 2 would be that they are placed there because that is where pedestrians are commonly crossing the road.
in France and Spain the crossings seem to be away from junctions and they work ok...here you get a couple of people cross the road and cars build up on these little roundabouts that appear overnight!
