What do I do to rectify this mess and also, what causes it to happen in the first place?

Is it anywhere else on the walls?
What paint finish is it and what finish is underneath Matt or silk.

It can happen when applying Matt paint on top of a silk finish, when first painted it can craze (orange peel affect). Its a case of bad adhesion between the two really.
 
Yes it's apparent in other places to. Not sure if there was silk underneath but it's a possibility (I didn't decorate the house). When I peel it off it takes the surface back to board.

Do I have to strip the whole wall - if so how the hell do I do that without having a massive task?
 
You will first need to scrap away the loose and give the areas a good sand then feather out the raised edges with filler
 
As its by a rad my first thought is you're drying clothes on it, so you're basically steaming the paint off. Follow the advice above to fix

Ah, didn't realise you could risk that with clothes drying. How do you feather the filler?

basically you work the filler into a tapered edge. If you don't get it right the first time then you can always give it a little Sand down.
