IHNJ is denying the advances in medical science which show that this:

but nature dictates gender (at birth), if you're born a male, you'll be a male till you die.
Is neither as straightforward nor as binary as he insists that it is.

So it is relevant to ask him where he wants to draw the line on what advances in medical science he considers valid, and which not.

I wonder if he has a great deal of expertise in this area, or whether he is simply rejecting that of those who know more than him because he doesn't like it?

Not being a man of science, I can say I have no expertise in the field of genetics, but I have never denied the advancements in medical science, they can even clone mammals now, whether its a good thing to clone a human is a different thing. But I stand by my statement that if you're born a male, you are male, cosmetic surgery will never change that.

There's more to it than a bit of cutting and sewing. Would you tell this "lady" to put a dress back on? To his face?

What about animals that change gender. And, some humans naturally change gender after birth. Rare, but it does happen. All down to genetics. Rare, any scientists do not really understand why it happens, but it does happen, and some think it is becoming more common. You're missing the point I am making, gender re assignment through surgery does not alter the gender, same as putting a body kit on a Ford Mondeo does not make it a Ferrari.
 
But it is never just surgery, is it? And gender can be changed hormonal. We all start equal, gender is determined by hormones. Hormones can be changed.

Biology is very interesting. You don't have to be a boy if you don't want to. Welcome to the new age.
You can have surgery to alter your looks, take hormones, you can do all sorts of things, except cheat nature, said "boy " will never give birth, and the boxer you linked to will never produce sperm to create a life. Many women can batter (& better ) a man, they don't need surgery for it.

The reality though, is that there are people who are born who believe, for whatever reason, are of a different sex to their body. Therefore gender re assignment is a process to alter there physical body to match the sex they feel they are mentally.
In which case perhaps one could argue, the person was born with the wrong body kit and they just replacing it for the correct one.

Rather like being gay, people that have been born into the wrong body cant help the way they are, its not a lifestyle choice, it is the way they are wired.

Are you not saying that, really, they have been born with the wrong (type of) brain?

Of course nothing can be done about that - but even if it could, it would be considered contentious, unethical and tantamount to eugenics, but altering the body for some reason, is not.
 
Thats before you start on the "non-binary" brigade,who do not see themselves as m or f!.
