When undertaking a rewire for say a house, is it usual practice to replace all the cables? Is there a case for retaining any cabling? Does a rewire include adding and/or moving existing sockets/switches.

Depends on what you agree with the customer.

Normally you would replace EVERYTHING electrical in the house from the meter tails, right upto the flex drops

Positions of the accesories is entirely upto the customer, unless their position would not comply with the regs (a switch in a bathroom for example)

thats usualy waht a rewire means

as for extra / additions thats up to what the client wants, its easier to do from scratch than to add later

Should one re chase all the new cables in or can you use the old conduit if it is possible to pull new cables through.

Hi,

If you're lucky then you may be able to pull cables through the existing conduit but in many homes these are not used and the cables may just be protected by capping behind the plaster or just buried behind the plaster and you'll find you cannot pull them. Also very few feeds to sockets are in suitable conduit to allow you to pull then through so you'll probably need to chase these out.
