It looks like a bit of a c@ck up
but just trim over it
one bit of 16mm x 16mm timber onto the flooring then another bit flat strip timber say 20 x 10 over it and butted up to the back of the door cill
a carpenter would just make up single piece with a rebate

exactly that
the bigger piece can be screwed with some 3.5mm x 30mm csk screws
the top bit could be fitted with some super glue and activator, or pins or small screws and fill the holes.
I would put a chamfer on the top bit -with a sander or block plane etc
its not the most ideal solution, but I think its the easiest option for you which wont look too bad.
that bit of brickwork with dpm over it isnt really very clever -it shouldnt have been done that way, but hey it is what it is
