Adaptor for old bathtub tap.

Hi,
I’m not an expert.
I would like to add a shower hose to this tap. As you can see, it has a strange shape. Is there any kind of adaptor? It looks like it could be unscrewed, but I wasn’t able to. Do I just need to use more force?

Swap tap with one that has a shower hose is going to be your best bet.
You will be available to control temperature and flow ect.
I think you are going to struggle or be disappointed even if you were to attach a hose to that tap
 
Cant say I've ever seen a diverter that would fit that shape. As suggested replacing the tap would probably be the only sensible option
 
Thanks. I will contact a plumber then. I might have to flip the orientation of the bathtub anyway.
