Anyone got a safe isolation procedure diagram

Does anyone have a safe isolation procedure flow diagram they could give me a copy of? I need it to put in NVQ3 portfolio.

sorry its for a electrical portfolio i'm doing. I just need a copy of safe isolation ie testing, lock off supply etc. I did it at college but require a copy off it. Just wondered if anyone had it they could pass it on to me. Thanks
