bathroom lighting help please

i have a low bathroom with a low ceiling - any recommendations on a suitable light fitting. not much room between plasterboard and roofing tiles (4 inches). light switch is outside bathroom in the bedroom

100mm between void is fine for low voltage downlights, however you mention low ceilings. How low are you talking off. Stange to have the switch outside unless this is an onsuite. If not i would install on a pull cord.
 
Is this a sloping roof? If it is I'd be tempted to put a false horizontal ceiling at the highest point with the wiring above. You don't need much width for a fluorescent light fitting and a pull cord switch.

PS: If you have a 4" void between the plasterboard and the tiles why isn't it full of insulation?

because its old (conversion done in 1940's i think)
i was going to take down the wall covering (tongue and groove) and properly insulate roof. however, i think i would need building regs for this which would then result in me having to redo the whole bathroom i worry. so i decided to leave as is. is this correct interpretation ?
