I've not seen one but have built over many sewers. I think it mainly deals with the gap around the drain and the drain itself. In other words, if your lintel clears the pipe by say 100mm and around the sides and if you renew the pipe in plastic with correct couplers, correct granular bedding etc, then you pretty much have it sorted.
Some water companies require a Build Over Agreement to be signed. It would cover access for maintenance, liabilities during constructions, damages to pipes, raising of the manhole etc .
Id like to have a look at a sample of the agreement if possible just to get an idea of what I should look out for and what to expect
