Yes to all really, it could have been done a few ways to get what we are seeing and what the OP tells us.
It really needs sufficient testing of the what ifs and whys to get a realistic understanding of what was done then what might have changed, if anything, too see how its ended up as what we have in front of us.
Never assume anything because assumptions may be misleading (but assumptions might give us a clue or part thereof).
