drilling wall help

I'm trying to put up a wall TV stand thingy and everytime i drill into the wall large chunks of it fall out, what am i doing wrong?

It would help if you gave the age of the house as well as a description of the stuff breaking out. My parents house was built right after WW II and had internal walls of breeze block. It was all but impossible to get even a toilet roll holder to fix safely to that stuff as it had the constituency and strength of a Crunchie Bar.
