kept under the immediate supervision of the person carrying out the work, means for securing
in the disconnected state need not be provided.

You're quite probably right about this thread, but, as far as I am aware, we never actually discovered anything about the nature of 'the machine' or what sort of environment it existed in.

The OP did state "Also, we have machines hard wired from the panels in the roof void with only an isolator on the machine itself, I just put another circuit breaker in on top of the the one the socket has up stairs in roof void." which does sound rather like a factory environment.

Just to be awkward - what if there were two or several sockets nearby.

Wouldn't it be better to lock the plug?

Isn't that what bhm1712 was suggesting? As you imply, if one is going to 'lock' sockets, one would have to lock every one within reach of the cable (and check that there were no 'extension leads' around!).

So with the machine effectively hard wired to the supply (if it appears that the local breaker is not there/bypassed), and we assume the machine is on fire or somehow the breaker on the machine is inaccesible - you still have emergency switching operations remote from the machine. It might well mean having more machines powered down, perhaps a whole column of them or the entire shop floor, but you still have the option of de-energising the entire board on which the single machine sits. If the machine is on fire and a firefighter is getting in there with a hose they what the whole lot isolated anyway.
