loose connection?
loose wire inside, shaking as the fan goes round.
If you bought it new from a shop, take it back and complain.
Otherwise, you, or someone, will have to cut the power to the lighting circuit at the consumer unit, take it down, open it up on the kitchen table and check for loose connections or damaged wires.
Or throw it away and buy a new one.

Could be a loose connection between switch and fan also.....
