Worcester recommend you change the complete valve not just the actuator.
Its clearly still passing in HW mode.

Carry on changing bits until it stops what it's doing. It will at least keep you occupied.

No offence taken on my part, but I think you are now wriggling, you were hardly specific about it being any particular monkey, and you probably meant it as originally written. Unfortunately now preserved in quotes. Why would it have been the fault of the commissioning technician? If it was that recent, the manufacturer's warranty would cover it.
