What reasons would cause a pilot light to go out when you release the button? (permanent pilot)

Apart from faulty thermocouple / tip dirty or not positioned correctly in pilot flame, fault overheat assembly/dirty contacts, what else is there? Faulty gas valve (pilot) solenoid?

Why are you asking and then answering your own question?? is this some sort of test? BTW you need to add "poor Pilot flame" to your list of answers
P.S. good luck with the ACS assessment!
