I have a peice of MDF; 1000 x 1000, and 18mm deep. I want to take the edge off on all 4 sides (as per the pic).

If I use a sander I'm not sure it will be perfect finish, Or will it?
Do I need to router it off, or do it some other way?

Thanks in advance.

Consider using a smoothing plane to take most of the waste off, hold it at 45 degrees. After that, you can sand the rest away using either a machine sander or hand glasspaper. Start with say 80 grit and go finer.
MDF dust inhalation is best avoided!

if its only a 1 off job,AND you do own a router then buy a cheap bit,it will do the job just fine.only a light sanding afterwards.
if you dont own a router, then as already mentioned either a block plane or a smoothing plane will do the job,if using a plane?what i would suggest is using a finger gauge you run lines on the sides you want the moulding to be,that way you will have a sight line to follow and keep it uniform.
