Siting a Shower Tray: How is Waste Managed?

Latest daft Q! I have a shower tray in an en suite which is about 4" off the floor. It works, but is jaded and I'd like to replace it. The current waste is above floor level. How do plumbers site a tray on the floor, and deal with the waste? I think underfloor will be a no-go, 'cos the waste would be going across joists. Any advice?

To get a tray at floor level the joists have to be running in our favour or run the pipe underneath and lower the ceiling below.
