They are either the cheaper pollytop nail or they have been driven home too far.
Sounds like they used screw cap covers
Polytop Nails heads come off for one of two reasons;
1. The nails are cheap. Make sure they are A4 Stainless from a reputable supplier.
2. They are not being hit squarely. Even the best polytops will lose their heads when hammered off centre.
