Transition for LVT flooring

Hello

How to I deal with a large transition, length (4cm) and height ( 1.2 cm).

I want to put LVT flooring in the kitchen and hallway. The transition is between the kitchen and hallway.

The kitchen is connected to the living room. The kitchen and living room are at the same level. This means I cannot level the kitchen with the hallway witout leveling with the living room too, which I don't want to do

Looks like it needs grinding. Does it have a damp proof visqueen under the screed
 
Yes the damp proof membrane sticks out the side of the room
