Hi

Would you say this is badly infected woodworm in this lintel?

Its a big oak lintel, so would it takes years for it to become a structural issue?

How best to treat it?

See holes they are gone.
Yeah. Treat it with wood worm solution. DIY sell.
Question.
You see dust if active. Seen any? New holes?
Might be historic and already been treated?

Unfortunately Ive seen new holes, and frass.

I wonder if I have to sand the stain off the wood before applying the treatment?
