Back door leaking.. Please help

Hello. I'm concerned about my backdoor as we have had some heavy rain in the last day and I noticed a wet patch in the concrete that meets the corner of the door frame. Somehow water is making its way into the house through both edges of the door. I have always had concerns over the effectiveness of the metal threshold and ability to stop rain coming in but I now have proof. I have taken lots of pics and am wondering if anyone can spot any potential areas of concern where the water could be coming in. and whether replacing the existing threshold is easy to do and what to replace with. Any help would be greatly appreciated as I can't lay my new flooring until this is fixed. The drain holes in the threshold are not blocked (tested today).

First things first , are the drainage holes on the threshold clear and letting water out?

Yes, I've just tested by pouring water onto the the threshold and water comes out of both drain holes.
