Seeking the community's help in identifying the make and specific model no's - of the parts in the pics.

Hoping for a) Manufacturer b) specific model number.

My Aim: To simply take off the old ones (hinges), and swap them out with fresh clones.

Maker definetly Trojan . Looks like Trojan 3D Patriot hinges to me ... recently changed some out for exact same problem

The door was fitted about 10 yrs ago.

What are the chances - of a straight forward, simple swap?

Fairly good. Look up the patriot hinges and then double check measurements, pretty sure it will be a straight swap

Just to confirm the mounting design changed significantly and the new version is not a direct replacement.
