I know an old thread, but I was in this situation a few years back. I had a big desk and put an oil filled radiator under it - found it to be much nicer than the fan heater and would warm the desk itself as well as my legs. Great for productivity too as it meant I spent more time working and less time sitting on the sofa having tea breaks.

If extremities getting cold then could be blood circulation issue... suggest getting tested for Diabetes (seriously)
