Downlights can be pointed where you want them or these days get the wider angle lights to spread the light rather than have them focused on the floor

The problem is no matter what, they are approaching a point source, so you still get heavy shadows cast even if you have a wide angle lamp. If you spread the light over a larger area to start with you get much more effective illumination.
