That particular box should be left blank but to be honest I'm a not that sure what you're talking about. It would make perfect sense if I could see this certificate though as I use ECA certs.

New2Game, these are the definitive anwers for you. (I guarantee it)

Designation of consumer unit is as Spark123 told you; it's whatever 'name' you want to give it.

The first row on page 3 is for details of the circuit supplying this particular consumer unit if it is remote from the origin. For example the 'designation' might be 'Garage' or 'DB2', in which case you can use this line to supply full details of the distribution circuit supplying it. If it is at the origin (next to the cut-out, as is usual) this entire line is N/A.

Get yourself a copy of the NICEIC's Inspection, Testing & Certification handbook, which gives you a box-by-box breakdown of exactly what goes where.
