Recessed manhole cover size - 685X535X80mm

Hello all, I am struggling to find a recessed manhole cover to replace my existing one which has a rusted and mashed lifting eye. The internal frame size from edge to edge is that is 685mm long x 535mm wide and the existing cover is 80mm deep. The existing cover is filled with block pavers and has sunk and the cover flexs when you stand on it. I suspect the previous occupier didn't use a concrete base and the sand has just washed away over time. I'm concerned that if I refill it correctly that the rusted and mashed lifting eye will give way and I won't be able to lift the cover if needed.

Can anybody offer any advice as to where I can buy one.
