Depends how far you want to go building wise, as the Airbrick is in the way

Not a builder sorry, but at a quess you could CAREFULLY whack it out 
put in a solid fan pipe which is roughly 100mm or so that the fan slips inside, brick up and make good round the pipe, taking care not to drop the muck down the wall cavity

The problem in your case is that doesn't appear to be much brickwork there to support the pipe.

It's tempting to forget about the air-brick altogether, and use flexi-duct out of the soffit seen in the picture.
