Help changing light bulb

Hi all. Need a bit of help changing my shower exhaust light.
It has the exhaust around the light, I've taken off a cover, which is pictured, and there then appears to be the screws on the bulb, and a kind of exhaust surrounding it.
Does the whole exhaust unit come out or do I unscrew the middle bulb bit?

You've taken the lens off the lamp.

I would imagine you just grab the silver bit with fins and twist it slightly to the left or right and it should drop down
Thanks for your reply. So I would need to buy the whole fin/lamp unit. I've never seen them in any diy/electrical store

I just can't get the thing out now. Pushing in and trying to twist anti clockwise, but it won't come out.
I imagine this is a 12v lamp, not a GU10, as it's a shower fan light, so as said above, it should just pull out.
