Hole in brickwork

I've been fitting some new loft insulation and have noticed that in one part of the loft there is a hole in the brickwork where day light is visible. I would estimate this hole to be about the size of a pound coin. The loft is on two levels and I cannot get closer than 5 metres to the hole to investigate further.

On the outside of the house I think I have identified the hole. My question is how can this be repaired, is it a case of re-pointing the brickwork on the outside or is interior access required?

This is why block/brickwork isn't air tight and people in dot and dab "plasterboard tents" are so cold (or poor)!
