attach the MCB securely to the busbar...

had he forgotten to wind the screw to the "fullly open" positon before putting it onto the busbar?
 
Busbar is also back-to-front.

The end with the holes is intended to be at the 'low load' end.



I would hazard that the busbar was the wrong side of the 'vice type' terminal.



Shower is on the RCD side.




Good old MEM Memshield 2000.
 
I have seen busbars mounted on plastic pillars in some makes of CU - GE spring to mind.
