I want to put a new socket under my worktop for the fridge, do i need to have a fuse above the worktop or can i just run a socket off an existing socket

If the socket is accessible under the worktop then u are OK. you should not be putting the socket where you cant get to it so put it in an adjacent cupboard, not behind the fridge.
I am assuming the existing socktet is not already a spur?

No i dont think so, the electrics are old.
Might decide to just run a new socket from the main, but was just checking because this is a cheaper option
