What is stopping anyone have 2 spurs at the same place?

It is no different to having a row of 3 double sockets in a row on the ring and how many times have we seen a group of 3 or more such sockets in one location for IT or AV type setups?
 
I don't know, but is this another case of
... it is common, in many fields, for people who have been following instructions or rules or whatever to see what they think is there, not necessarily what is there, particularly if early on they misinterpreted something, or the person who taught them got it wrong, and so on.

And do they also state that it will accept 3 x 4mm²?

If they do then it will easily accept 4 x 2.5mm².
I don't think I've ever seen an accessory vendor rate an accessory for more than three conductor in the same terminal. Even when there would appear to be space for such. For example MK rate most of the the terminals on their FCUs* for 3x2.5mm² 2x4mm² or 2x6mm², with the additional stipulation that if 2x6mm² are used they must be stranded.

I suspect this is because as the number of wires increases the chances of ending up with an unfavourable geometry that does not grip all the wires solidly increases, especially as the insulation on the wires will tend to push them apart.

* The one exception seems to be the load side earth terminal which is only rated for 3x2.5mm² 2x4mm² or 1x6mm²

If they don't and it will not then it is not fit for purpose in the U.K.
It's a while since I've read BS1363 but I'm pretty sure it only requires 2x4mm² though in practice most manufactuers exceed this.
 
Plugwash, I take onboard what you have said although I am struggling a bit to envisage "the chances of ending up with an unfavourable geometry that does not grip all the wires solidly increases" scenario whilst using proprietary UK manufacturers goods.
I reckon those state the minimum most folk should be able to achieve, yet realise that the more experienced should be able to improve on that without too much trouble.
Just my opinion.
