Looks like whatever happened was in relation to the 3 phase C40 MCB, was the isolator not fully over? Loose connection? Were those 3 phase wires really routed through the middle of the dis board?
It looks to me as if the short and subsequent arcing was on the three bus bars behind the labels.

I was a dead short accross all 3 busbars, blew all 3 DNO fuses. That much crap fell out of the board when it was took down that we were unable know for sure what caused the short. All we did that night was get the landlord's accomadation back on, the rest was done the next day.
Good old mice! They can sneak in through the smallest of gaps left in a bush full of cables! They bring in anything and everything.......
