They can be billed for any care(and including additional costs) they receive if they're not covered by their own insurance.
It's up to UK to chase that money owed.

If they're EU citizens they'll have a card similar to the GHIC Uk card.Treatment is provided on the same basis as the indigenous people receive.
The government makes the rules in conjunction with other card providers.
The government can change those rules by exiting the GHIC scheme and look for another.
It's not the fault of the foreigner needing medical care.
In addition if they have travel insurance, they'll be covered by that. But the UK government has to chase it.
