Removing combined downlight/extractor

As you can probably see, my en suite ceiling needs sanding back and replastering.

Is there a clever way or knack to remove these combined downlight/extractors from the room? It’s in the apex of a roof so /very/ difficult to get to the other side of it

When you say 'in the apex', do you mean in a sloping ceiling below the actual roof tiles/slates?
Or is there a standard sort of loft above?
 
The white outer ring unscrews, usually clockwise which is the opposite to what you might expect.

Removing that will reveal screws which attach to plastic clamps on the other side of the ceiling.
