In my terraced house, typical 2 up 2 down with stairs up the middle, the floors at the back of the house are sloping away from the middle of the staircase - ie. sloping from centre of house to the back door. The slope is evident both upstairs and downstairs.

I believe this is due to historical movement as these are old terraced houses.

I was going to rip up the floor, and sister the joists, however in the kitchen, if i levelled the timber joists by sistering, the top of the joist would probably be higher then the bottom of the external door frame.

What can i do to correct this?

Is it a case of rip it up and see whats happening?

Often a step down to rear door in old terrace properties, is yours flush ?
One option may to replace timber with solid floors then height problem is no longer .

as the fiinished floor in the kitchen meets the frame of the external door, there is only a height of approx 3-4cm, which is the pvc frame of the door.

In terms of the floor, the floor seems to dip more at the point where people are standing when opening the door. To me, this indicates possibly a joist warped, although the drop is pretty significant. I will have to measure the drop over the span of kitchen. Although with it being upstairs and downstairs, i assume subsidence
