Well the UK could build an elaborate infrastructure of CCTV with facial recognition software that could identify each person and associate them with a record on a national database to see if they are bona fide citizens or illegals.

Then the system could be (ab)used for all sorts of other things.

Or they could enact legislation to make other citizens perform all sorts of checks before employment, health treatment, services, schooling, etc is allowed.

The are SUPPOSED to carry out an id check - could be a lease, a rent book, official letters to them at that address etc. The GP can then take them on to their books. The minute they are taken on, they are given a NHS number FOR LIFE. Dodgy doctors looking to fill their books and get payment for patients are sometimes 'lax' with the id check. These are generally NHS tourists that you hear about. The true illegals - car washers, sex workers, nail bar girls etc don’t bother with GP surgeries.

Yes, my wife worked as a receptionist in a GP surgery. First hand experience although you would never believe any examples I give you so I won’t waste my time. Luckily they were red hot on that sort of thing as well as taking people from outside their catchment area. You wouldn’t have been able to move out and still remain on the books in their surgery because "it was convenient" to you. It has to be convenient for the GP.

You have two types of doctors. Good, straight ones who really are there to care for their patients and some rotten ones who are there to rake in as much as they can. A system like that will always attract those that want to abuse the system both for those that run it and those that use it. No different from crooked solicitors, bankers, estate agents, landlords, electricians, builders, plumbers, roofers, mot testers, garages........virtually nothing is unabusable.
